Considering that makers of the vihuela and the lute and other guitar predecessors had been toying with 'double courses' for centuries, the maker of the first 12-string would be highly debateble.

The answer would have to be that nobody knows for certain. Some people say it was invented in Italy, while others say it came from Mexico. It may even have originally been invented somewhere else.
