With all due respect to "The Usual Suspects" and "Mullholland Drive" (great films with great twists) my vote goes to "The Kremlin Letter". If you don't mind watching a movie that's more than a few years old, I think you'll find that this movie has so many twists that the final one catches you completely by surprise.
Am I the only one who thought that "The Sixth Sense" and "The Skeleton Key" telegraphed their endings?

Deffinately 'the usual suspects' - it sprang instantly to mind while reading your question, what a payoff, couldn't stop smiling at the end of that film, soooo good

The end of 'The Sixth Sense' STILL gives me shivers -- it was SO cool!

But that scene in 'The Crying Game' was a HUGE shock too!
