My vote is for Natalie Merchant's "The Living", from her "Ophelia" album. It is about suicide. Can't get much sadder than that.

There are a few more I could throw in there too, such as Marianne Faithfull's "Ballad of Lucy Jordan" or Emmylou Harris's "Red Dirt Girl", both which also take on the story of a suicide, but none quite compare to the utter bleakness of Natalie Merchant's song.
