Does anybody has a sheet music for The Prayer of Saint Francis? And who wrote it anyway?

The part of the lyrics is "Lord, make me an instrument...." and so on. I've only heard it once and it's so beautiful.

Lord,Make Me An Instrunent Of Thy Peace....found in several newer church hymnbooks...I'm looking at "The Singing Church" but there are several others containing this hymn....usually called by the first line in the index.
